Why can't I log into MyChart? ›

If you try to log in unsuccessfully too many times, you might be locked out of the account. In this case, you won't be able to reset your password yourself. For help resetting passwords and regaining access to your account, please reach out to your healthcare organization's MyChart support desk.

Your activation code will be printed on your after-visit summary, can be printed for you or sent to the email address we have on file for you, upon your request. You will not need to use this code after you have completed the sign-up process.

Most likely, you have tried to access your MyChart account 5 times in a row using the wrong password. If you do this, for security purposes, your account is disabled. If your account becomes disabled, you should call 740-454-5008.

What happened? We aim to protect your privacy and information. If you remain idle for 10 minutes or more after you log in to myChart, you will be automatically logged out. We recommend that you log out of myChart if you need to leave your computer for even a short period of time.

MyChart is Epic!

MyChart gives you access to your health information and a direct connection to your care team, with tools to help you actively participate in your care. What is Epic?

Multiple members of your care team may view MyChart messages in order to get an appropriate and efficient response; this may include nurses or advance practice clinicians other than your primary care provider (PCP).

Can I link them together? MyChart accounts can be linked so you can see all your information in one view. We call this Happy Together.

While you will no longer be able to access your account, your medical information and other associated data will not be deleted for legal requirements and to ensure that you continue to receive the best possible care from your healthcare providers. You will not be prevented from signing up for MyChart in the future.

Why is my MyChart activation code not working? ›

For your security, your activation code expires after a set period of time and is no longer valid after the first time you use it. If you still have problems, Optum Care Washington patients can contact MyChart Customer Support line at 425-258-3904.

An activation code (also known as an activation key) is used to activate the software after installation. Activation ensures that the software is genuine and legally licensed. Unlike a product key, which is entered during installation, the activation code is applied after installation.

Reminder: If you receive an email that your account has been deactivated, please contact the MyChart Customer Service Center 8 a.m. to 4:30 p.m. weekdays: submit an online form to reactivate your account.

If you are unable to recover your MyChart Username, or you do not have a valid email address on file, please call the toll free MyChart patient helpdesk at (877) 768-0732 to regain access to your account.

To access your child's MyChart account, you will need to set up your proxy access. This means you, the parent or guardian, must have your own MyChart account first. (Please note: foster parents cannot have proxy access to their foster child's MyChart account.)

What do I need to use MyChart? You need access to a computer or mobile device connected to the Internet and an up-to-date browser (such as Edge, Chrome, Firefox, or Safari). You can also use the MyChart mobile app on a mobile device.
